Our team is proud to be an integral part of the health care delivery system that reduces financial, cultural and physical barriers to meet the needs of the community. 

We provide the following support services:

Care management/coordination

Our Care Management Team will assist patients and their support systems in managing medical conditions more effectively. We believe in reducing health risks and the cost of care by delivering the most appropriate care to the most appropriate patients, at the most appropriate times.

Community outreach

Our Community and Patient Engagement Team connects our communities to a wide array of health services as well as domestic violence services, social services, and community partners that can provide assistance or care. The CAPE team can provide health education services and perform health screenings at your next event. For more information visit this webpage.

Insurance enrollment

We will assist you in applying and enrolling in medical assistance. Our team of Certified Financial Counselors with complete and submit your NJ FamilyCare applications all year long and will handle Marketplace applications during open enrollment ( November 1- December 15th)  or year round if you have a special qualifying event. Prescription assistance

If you can’t afford the prescription drugs you need you may be eligible for free or discounted medicines through our prescription discount programs. As a 340B participant, our patients have access to outpatient drugs at reduced rates. Southern Jersey Family Medical Centers patients are also eligible for prescription assistance made available by pharmaceutical companies. Patients must qualify and apply directly with drug companies.


At SJFMC it is important for us to provide care to our patients in their primary language.  We have several lines of support for translation and interpretation services. From bilingual staff including providers to translation technology, we work hard to eliminate linguistic and cultural barriers to care.


Transportation plays a vital role in one’s ability to live healthy. Lack of transportation is a major barrier that contributes to delayed or missed appointments, inability to comply with follow-up health management care plans, increased emergency room visits, and overall poor health outcomes. At SJFMC our transportation team will pick you up so that you can get the care you need.

Services are provided to all patients in a sensitive and culturally appropriate manner, regardless of their ability to pay. Uninsured patients are welcome, and outreach efforts are designed to reach patients most in need of comprehensive and accessible medical care.
