***You must be 18 or older and live, work, or study in New Jersey to schedule and receive the Moderna vaccine at SJFMC.***

***If you are are moderately to severely immunocompromised and scheduling a third dose appointment you must receive your additional dose of an mRNA COVID-19 Vaccine at least 28 days after the completion of the initial mRNA COVID-19 vaccine series. Please bring your vaccine card to your appointment.*** 

***If you were fully vaccinated early in the vaccination rollout, you can pre-register now for an appointment on or after 9/20/21.
