Available Healthcare Services
SJFMC physicians, practitioners, dentists, nurses and other support staff provide a wide range of services including:

Pediatrics/Child Health Care -
SJFMC provides timely immunizations and well-baby examinations. We also focus on patients’ physical development and common childhood illnesses.

Adult and Family Medicine
Adults of all ages receive annual physical examinations, ongoing treatment for chronic diseases, such as high blood pressure, diabetes and diseases resulting from HIV infections. In addition, SJFMC offers annual flu shots and vaccinations to prevent pneumonia, as well as referrals for x-rays and specialty consultations.

Women’s Health Care (Obstetrics/Gynecology)
Women’s health services are among the most frequently used services at SJFMC, especially by women of childbearing years, between the ages of 15 and 44. Sexually active teens and women need regular check-ups, annual physical examinations and cervical cancer screenings to help stay healthy and prevent disease. SJFMC also provides comprehensive prenatal and postpartum care for pregnant and parenting women. Such care increases the chances that healthy babies will be born.

Dental Care
SJFMC’s primary and preventive dental services assess and treat tooth decay, oral cancers and gum disease. When dental care is a part of the primary health of our patients, it is possible for patients to keep their teeth most of their lives.

Foot Care
Healthy feet are important to all patients, but are crucial for migrant and seasonal farm workers, pregnant women, workers who stand on their feet most of the workday and for senior citizens. Problems associated with diabetes and infection of the feet are addressed by our foot-care specialists.

Nutritional Counseling
Nutritional deficiencies rob the body of its own natural resources and can manifest as symptoms such as fatigue, mood swings and insomnia. Many times, these symptoms are taken for granted as a natural sign of aging. When left unchecked, however, these same deficiencies can contribute to disease such as arteriosclerosis, high blood pressure, diabetes and arthritis. A nutritional counseling session will provide you with specific information that is tailored to your individualized wellness needs.

Clinical Laboratory Services
Health care providers often prescribe diagnostic tests for patients in order to obtain information required to make more informed medical diagnoses or to determine abnormal blood levels. Clinical laboratory tests include blood tests, body fluid testing, tissue pathology, health screenings, drug screenings, and genetic-based testing. Once laboratory test results are submitted to SJFMC, you will be contacted to return to the center to receive the results of your tests or results may be given to you by telephone. Patients will be contacted immediately for abnormal results. Your doctor will interpret your laboratory test results and provide you with medical advice, diagnosis and recommended treatment. Some lab services are performed at the center. Additional costs may be incurred. Lab results are available from 10am – 4:30pm.

Prescriptions
Renewals are available at time of service. Medication renewals will not be approved without a recent doctor’s visit. Please allow 4 hours to process.

Work/School Notes
Our physicians will provide written documentation of your visit. We ask that you make your request for a work or school note within 24 hours of your visit.

Records Release/Completion of Forms
Patients are required to sign a release form at the center to transfer or obtain a copy of medical records. Duplication of records can take up to 48 hours. Forms may not be completed at the time of the visit. You will be notified when your form(s) are complete.

Referrals
If you need healthcare services not offered by us, we will evaluate your needs and discuss them with you and your insurance carrier. If approved, your doctor will authorize a referral. Southern Jersey Family Medical Center will require at least 24 hours to process referrals.
