and Insurance Coverage for Services
SJFMC’s mission is to provide patients with access to
comprehensive primary and preventive medical, dental care
and ancillary services. Our staff will assist you in determining
if you are eligible for various publicly funded programs.
If you have no health care coverage, we will require you to
provide certain documentation in order for us to qualify you
and your family for care. If you have some form of income,
our staff will review our sliding-fee scale with you and payment
will be based on your financial ability to pay for those services.
