Prescriptions
Requests for prescription refills can be made during your office visit or via telephone. All telephone requests must be made within 90 days of your last office visit. Please allow 24 hours to process requests.
Drug Discount Program
SJFMC is a proud participant of the 340B Drug Discount Program. Through this program SJFMC can provide our patients with access to certain discounted prescriptions. Please contact your health care provider for information.
Pharmacies in other SJFMC neighborhoods will be available in the near future.

Work and School Notes
Our physicians will provide written documentation of your visit. We ask that you make your request for a work or school note within 24 hours of your visit.
Records Release / Completion of Forms
Patients are required to sign a release form at the center to transfer or obtain a copy of medical records.
Please allow 7-14 business days to process the medical record request. Forms to be completed by your healthcare provider can be mailed or dropped off at the center. Please allow 7 business days for the completion of the form. You will be notified when the form(s) have been completed.
Physician Referrals
If you need healthcare services not offered by us, we will evaluate your needs and discuss them with
you and your insurance carrier. If approved, your doctor will authorize a referral. Southern Jersey Family Medical Center will require at least 24 hours to process referrals.
